I'm really glad that you liked the plot bunny, and I wasn't expecting such a positive response. It made me so excited to hear that you might write it! By the way, I'm a biomed major, and I've been thinking more about the plot bunny. I thought I'd share my conclusions with you, and you're free to use this information or take it with a grain of salt.
I would also like to apologize ahead of time for my rambling and info-dumping. Honestly, I do not expect you to read through all of this, however, if you have any questions I'd be happy to answer them and clarify anything! ^^
Shěn Qīngqiū "hadn’t been taking the greatest care of his own cultivation", "had forgotten the suppressing drugs for Without-A-Cure for a day or two." and "He just couldn’t manage to get it down with the bland food on his peak". So, because Shěn Qīngqiū forgot to take his suppressing drugs for Without-A-Cure for a day or two, his meridians were more unstable than usual, which was made worse by Without-A-Cure. Also, he couldn't eat the bland food, which means he probably hasn't eaten anything much since Binghe was pushed into the abyss. This, even with Inedia, would have caused him to experience malnutrition. As a result, his cultivation could have become even more unstable, his meridians weakened, and his body overall weakened, which explains why he couldn't do anything about the rotting wooden pillar.
It's possible that Shěn Qīngqiū is on a tightrope between hypothermia (caused by Without-A-Cure) and hyperthermia (caused by the fever), which is extremely dangerous and rare, as the body usually tries to equalize temperature. Also, no matter what he came down with, it would likely turn into pneumonia. I thought about the gag and how reusing the same mask can make you ill. Although I haven't researched gags in detail, it's implied that he was gagged with cloth. If he had a fever and was forced to breathe through his mouth, the gag would already be soaked with saliva and likely be teeming with bacteria. There's even a chance of it being soaked with stomach acid, as he has had an empty stomach for so long. With a suppressed immune system, he'd have no defense against multiple types of infections - viral and bacterial - that might take him as a host.
If Shěn Qīngqiū had any injuries, there's a chance that he could get not only normal wound infections but also demonic infections. Depending on the severity of the injuries, he might even be close to developing sepsis. If the wounds aren't clotting properly, his torn robes would be soaked with blood, worsening the hypothermia and causing anemia. This could even lead to Hypovolemic Shock. This would not paint a pretty picture, and would heavily alarm Bīnghé.
I actually would not be surprised if Luò Bīnghé does not give Shěn Qīngqiū his blood during this fever purely out of fear of making it worse. However, I will also not be surprised if he gives Shěn Qīngqiū his blood purely out of desperation. He may also misinterpret (or correctly interpret) the wounds having been caused by the clan that attempted to "gift" Shěn Qīngqiū to him, causing them to be wiped out. I could also see Shěn Qīngqiū having mixed Delirium, meaning he would experience both hyperactive delirium and hypoactive delirium. The former is characterized by agitation, mood swings, combativeness, delusions/hallucinations, and a loss of filter during conversation. The latter is characterized by reduced facial expressions, speaking, and responsiveness to changes in the environment; apathy, sluggishness/lethargy, and similar symptoms to depression. This gives the possibility of revealing way more than he should, including the system which likely will incur a punishment. Bīnghé panicking would be a given.
It's also heavily implied throughout SVSS that Shěn Qīngqiū during his time as Shěn Yuán was sickly and from my own experience if he was bed-bound a lot- especially in a hospital- he would not get much in the way of physical comforting (hugs, cuddling, etc.), but a lot in the way of meaningless words. Possibly causing a negative reaction to any attempts at verbally comforting Shěn Qīngqiū, but acceptance of anything physical. That, combined with the line from the third chapter
"Shen Qingqiu pressed himself against each curve, briefly debating on if he could tuck himself partially under Binghe."
I could see Shěn Qīngqiū being extremely clingy in his delirium, possibly even showing agitation when Luò Bīnghé attempts to get up to prepare more food or medicine.
Of course, there's quite a bit more I could ramble on about, but I doubt you want a whole dissertation on this. However, I am happy to info dump and assist as requested. ^^
